building an innovative model of care across the Somaliland Primary Care Network,
designed to better utilize pharmacy professionals within the wider healthcare team. Our
mission is simple: keep patients safe, improve wellbeing, and reduce unnecessary
hospital admissions through smarter medicine use. Job Summary We are seeking
an experienced Clinical Pharmacist to join our multidisciplinary team. The successful
candidate will: • Provide
expertise in clinical medicines management. • Conduct
structured medication reviews (face-to-face and telephone). • Manage
long-term conditions and safer prescribing systems. • Oversee
medicines on transfer of care and repeat prescription processes. • Lead quality
improvement initiatives, audits, and medicines storage (including cold chain and
environmental impact). • Promote medicine optimisation principles to support specialist doctors and improve patient outcomes. This is a pivotal
role requiring motivation, innovation, and passion to deliver excellent service in general practice. Training will be provided for new and innovative aspects of the role. Main Duties • Drive
efficiency and effectiveness in prescribing activities across the PCN group. • Promote
high-quality, cost-effective, evidence-based medicine use. • Respond to
medication-related queries from patients, doctors, and healthcare staff. • Develop safe
and efficient medicines optimisation programmes (repeat prescribing, reconciliation,
reviews). • Use and
support IT systems (TPP/SystmOne) for accurate data recording. • Undertake
clinical medication reviews for stratified-risk patients (care homes, longterm
conditions, learning disabilities). • Recommend
medication optimisation strategies to senior pharmacists and GPs. • Encourage
self-care and management of minor ailments. • Contribute to
drug monitoring processes and alerts. • Analyse and
present medicines data to support decision-making. • Build
collaborative relationships across the healthcare communityJob search

Qualifications • Bachelor’s
degree in Pharmacy. • Minimum of 3
years’ experience as a registered pharmacist or in a similar role. • Strong
knowledge of clinical governance, safeguarding, and infection control. • Excellent
communication and teamwork skills. • Commitment to
openness, transparency, and patient-centred care. Personal & Professional Development • Actively
participate in internal and external training programmes. • Stay updated
with evidence-based practices. • Progress
